Bila da rasa sihat sikit, aku pun google tentang demam urat susu atau as Dr Khadijah inform nama saitifiknya, MASTITIS. Kata doktor khadijah simple je, "Namanya Mastitis. It is infection." Jadi aku mula berfikir dr mana datangnya infection?? Pengotorkah aku??
Sambung2 google, jumpa satu article by Elizabeth Jay yang bertajuk "Milk stasis – not infection – is the main cause of mastitis". Ringkasan dan point penting dari artikel itu adalah berikut:Mastitis, Infection or not?I thought that mastitis was caused by a bacterial infection. This is not completely unreasonable, given that this is precisely how numerous ‘health’ websites define it. When you consider that the symptoms of mastitis can include a fever as well as redness, lumps and pain in the breasts, and treatment can involve antibiotics, the definition seems to make sense.
Unfortunately, it turns out to be somewhat misleading. Whilst bacterial infection may play a part in mastitis, it is in fact inflammation of the breast tissue that is at the root of the condition, and causes the majority of the symptoms. The World Health Organization describes mastitis as ‘an inflammatory condition of the breast, which may or may not be associated with infection’

What cause Mastitis?
It appears that the inflammation that characterizes mastitis is a consequence of ‘milk stasis’: milk is produced, but then remains in the breast, rather than coming out during feeding. Milk stasis can occur for many reasons, including blockages in the ducts, a decrease in feeding frequency and poor attachment. It’s also possible that stressmight play a role, by both increasing milk production and delaying the letdown reflex. Why milk stasis goes on to cause inflammation isn’t so clear, though it could result from inflammatory substances found in milk irritating the breast tissue, or an immune reaction to certain milk proteins.

Itu artikel pertama. Surfing surfing lagi, jumpa buku pulak, maka faktanya lebih reliable.
Mastitis, Cause & Management. By World Health Organisation.

What is Mastitis?
Mastitis is an inflammatory condition of the breast, which may or may not be accompanied by infection. It can occasionally be fatal if inadequately treated. (Okey scary kejap!) Breast abscess, a localised collection of pus within the breast, is a severe complication of mastitis.
Awareness is growing that inefficient removal of milk resulting from poor breastfeeding technique is an important underlying cause, but mastitis remains synonymous with breast infection in the minds of many health professionals. They are often unable to help a woman with the condition to continue to breastfeed, and they may advise her unnecessarily to stop. (So maksudnya hati-hati jumpa health professionals. Ramai yang tak tahu tentang Mastitis dan ada yang akan nasihatkan utk ibu menyusu berhenti menyusu!!)

What cause Mastitis?
The two principle causes of mastitis are milk stasis and infection. Milk stasis is usually the primary cause, which may or may not be accompanied by or progress to infection.
Milk stasis occurs when milk is not removed from the breast efficiently. This may occur when the breasts are engorged soon after delivery, or at any time when the infant does not remove the milk that is produced from part or all of the breast. Causes include poor attachment of the infant at the breast, ineffective suckling, restriction of the frequency or duration of feeds, and blockage of milk ducts.

Baca punya baca, banyak kali dikatakan Mastitis disebabkan oleh susu yang'stasis'=motionaless=stagnant=tidak bergerak. Maksudnye susu tak dikosongkan dari ibu.
Aku teruskan pembacaan dan terjumpa Predisposing factors of Mastitis. (Faktor tambahan). Banyakla faktor2 ni, tapi yang aku rasa possible berkait je aku amik kat sini.
Immune factors in breastmilk may provide a defence mechanism in the beast. A study in the Gambia suggested that when levels of these factors low, effective defence may be reduced, and the risk of recurrent mastitis increased.

Stess & fatigue have often linked to mastitis but again there is little firm evidence
Trauma to the breasts from any cause can damage gland tissue and ducts and this could lead to mastitis. A possible cause that should not be overlooked is domestic violence, which affects many women in all societies, and is likely to occur during lactation.

So all mothers, kalau yang tak pernah ada knowledge tentang Mastitis ni, harap dpt gain something fom this entry. Jika anda pernah merasainya, jangan sedih sbb ramai ibu2 menyusu pernah merasainya! You have us!
